ABOUT THIS FACILITY
Golf Life 24/7 — The Practice Club is a private indoor Trackman golf simulator studio located at 12011 Nebel St, Rockville, MD 20852. The facility operates on a self-service model and is monitored by CCTV security cameras at all times.

TEMPORARY OPERATING HOURS (Effective Sunday, June 22, 2026 — Until Further Notice):
The facility is currently staffed and operating from 9:00 AM to 9:00 PM daily. These hours are temporary while final systems checks, operational setup, and remaining build-out are completed. Extended 24/7 self-service access will be made available once all systems — including the automated door lock system, booking platform, and facility operations — have been fully tested and verified. Members and guests will be notified when full 24/7 access is activated.

ASSUMPTION OF RISK
I understand and acknowledge that the use of golf simulator equipment, golf clubs, and related facilities involves inherent risks of physical injury, including but not limited to: Being struck by golf balls, golf clubs, or related equipment; Slipping, tripping, or falling on the premises; Muscle strain, joint injury, or other physical exertion-related injuries; Equipment malfunction or unexpected simulator behavior; Injury caused by other guests sharing the facility. I voluntarily assume all risks — known and unknown — associated with participating in golf-related activities at Golf Life 24/7, whether or not such risks arise from the negligence of Golf Life 24/7, its owner(s), employees, or agents.

RELEASE OF LIABILITY & HOLD HARMLESS
In consideration of being permitted to use the facilities and equipment at Golf Life 24/7, I hereby release, waive, discharge, and covenant not to sue Golf Life 24/7, its owner(s), operators, employees, agents, and affiliates (collectively "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me while on the premises or while using any equipment or facilities owned, leased, or operated by Golf Life 24/7.

This release includes, but is not limited to, claims arising from the negligence of the Released Parties.

INDEMNIFICATION
I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, cost, or expense (including reasonable attorney's fees) that they may incur due to my participation in activities at Golf Life 24/7, including any claims brought by third parties arising out of my conduct or the conduct of my guests.

EQUIPMENT DAMAGE RESPONSIBILITY
I accept full financial responsibility for any damage I or any member of my party cause to Golf Life 24/7 property, including but not limited to: Trackman simulator hardware and software; Golf equipment (clubs, balls, mats, hitting bays); Furniture, fixtures, screens, projectors, and technology; The physical structure of the facility. I agree to immediately report any equipment damage to Golf Life 24/7 management and understand that I will be billed for the full cost of repair or replacement.

GUEST RESPONSIBILITY
As the booking member or reservation holder, I am solely responsible for the conduct, safety, and actions of all guests accompanying me. I accept full legal and financial responsibility for: Any damage to Golf Life 24/7 property caused by myself or any guest in my party; Ensuring all guests understand and comply with facility rules prior to use; Any injury sustained by my guests while on the premises.

I understand that my access code (when applicable) is personal and non-transferable. I am responsible for all activity that occurs under my assigned door code and bay code.

24/7 UNSTAFFED ACCESS ACKNOWLEDGMENT
I understand that Golf Life 24/7 is designed to operate as a fully self-service, unstaffed facility available 24 hours a day, 7 days a week (once full operations are activated). During the current temporary staffed operating period (9 AM – 9 PM), a staff member may be present. I acknowledge: Outside of staffed hours, no Golf Life 24/7 personnel will be present on-site; The facility is monitored by closed-circuit television (CCTV) cameras 24/7. By accessing the facility, I consent to being recorded; In the event of an emergency, I am responsible for contacting appropriate emergency services (911) as no on-site staff may be available to assist; I agree to ensure the facility is properly secured upon my departure.

BYOB / ALCOHOL POLICY
Golf Life 24/7 permits guests to bring their own alcoholic beverages ("BYOB") onto the premises for private events only. By signing this waiver, I acknowledge: I am of legal drinking age (21+), or I accept full responsibility for ensuring no alcoholic beverages are consumed by anyone under 21 in my party; I assume full personal responsibility for my conduct and the conduct of all guests while consuming alcohol on the premises; Golf Life 24/7 and its owner(s) bear no liability for any injury, accident, property damage, or incident arising from alcohol consumption on the premises; I agree to indemnify and hold harmless Golf Life 24/7 from any and all claims arising from alcohol consumption during my visit or event; Golf Life 24/7 reserves the right to revoke facility access at any time if alcohol-related conduct is deemed unsafe or disruptive.

MINORS
If I am signing on behalf of a minor (under 18 years of age), I represent that I am the parent or legal guardian of said minor. I agree to all terms of this waiver on behalf of the minor and accept full responsibility for the minor's conduct and safety while on the premises.

MEDICAL CONDITIONS
I represent that I am in good physical health and have no medical conditions that would prevent me from safely participating in golf-related activities. I acknowledge that Golf Life 24/7 is not responsible for any medical emergency arising from a pre-existing condition.

PHOTOGRAPHY & MEDIA
I grant Golf Life 24/7 permission to use any photographs, videos, or recordings captured on the premises for marketing, social media, or promotional purposes, unless I notify staff in writing prior to my session.

GOVERNING LAW
This waiver shall be governed by and construed in accordance with the laws of the State of Maryland. Any disputes arising from this waiver shall be subject to the exclusive jurisdiction of the courts of Montgomery County, Maryland.

SEVERABILITY
If any provision of this waiver is found to be unenforceable, the remaining provisions shall remain in full force and effect.

ACKNOWLEDGMENT
By signing below, I confirm that: I have read this entire waiver and understand its contents; I am signing voluntarily and of my own free will; I am 18 years of age or older, or I am a parent/legal guardian signing on behalf of a minor; I agree to be bound by all terms and conditions stated above; I understand that this is a release of liability and a legally binding contract.
