1. ASSUMPTION OF RISK AND INJURY ON PROPERTY

I understand and acknowledge that golf and related activities involve inherent risks, including but not limited to being struck by golf clubs, golf balls, or other equipment; slips, trips, and falls; and other hazards associated with physical activity in an indoor golf environment. I understand that serious injury, including permanent disability or death, may result from these activities. I further understand that these risks exist whether I am actively swinging a club, standing nearby, or otherwise present on the property.I agree that my presence at and use of the Golf Life 24/7 facility, including all bays, common areas, restrooms, and parking areas, is entirely at my own risk. I expressly assume full responsibility for any and all injuries, losses, or damages of any kind that I may sustain or that may occur to me while on or about the property, whether arising from my own actions, the actions of others, or conditions on the premises.

2. NO ON-SITE STAFF; SELF-SERVICE FACILITY

I understand that Golf Life 24/7 is primarily a self-service facility and that staff may not be present on-site during my visit, including during late-night or early-morning hours. I acknowledge that I am responsible for my own safety and the safety of any guests I bring with me. I agree to use reasonable care at all times, to follow all posted rules and instructions, and to immediately leave the facility if I observe any unsafe condition that I cannot correct.I understand that in the event of an emergency, I must contact emergency services (such as 911) directly and that Golf Life 24/7 staff may not be immediately available to assist.3. USE OF EQUIPMENT AND FACILITY RULES

I agree to use all Trackman bays, golf clubs, balls, mats, nets, screens, furniture, and any other equipment or fixtures in a safe and responsible manner, consistent with their intended purpose. I will not misuse, damage, or tamper with any equipment or property.I agree to maintain the integrity of the facility by:- Refraining from any reckless, abusive, or destructive behavior;- Not allowing unauthorized individuals to enter the facility using my access codes;- Ensuring that no other unapproved people are present in my bay or in restricted areas; and- Leaving the facility in a clean and orderly condition after my session.I understand that any violation of these rules may result in suspension or termination of my access, and I may be held financially responsible for any damage I cause.

4. PERSONAL PROPERTY

I understand and agree that Golf Life 24/7 is not responsible for any loss, theft, or damage to my personal property or the personal property of any guests I bring to the facility. This includes, but is not limited to, golf clubs, bags, electronics, clothing, wallets, keys, and vehicles or items left in vehicles.I acknowledge that any personal property I bring to the facility is brought at my own risk, and that any property left at the facility, whether intentionally or unintentionally, may be considered abandoned and may be removed or disposed of at the discretion of Golf Life 24/7 without compensation.

5. ACCESS CODES AND FACILITY SECURITY

I understand that access to the facility is controlled by door and bay codes that are unique to my reservation or membership. I agree to keep my access codes confidential and not share them with any other person. I am responsible for all activity that occurs under my access codes.I agree to use the facility only during my reserved time and to exit promptly at the end of my session. I will ensure that doors are properly closed and secured when I leave.

6. VIDEO SURVEILLANCE AND SECURITY CAMERAS

I understand and acknowledge that Golf Life 24/7 may use security cameras and video surveillance throughout the facility, including entrances, common areas, and bays, for safety, security, and operational purposes. I consent to being recorded while on or about the property and understand that such footage may be reviewed and used by Golf Life 24/7 in connection with safety, security, investigations, or enforcement of facility rules.I understand that security cameras are not a guarantee of safety and do not replace my responsibility to act safely and follow all rules.

7. RELEASE, WAIVER, AND INDEMNIFICATION

To the fullest extent permitted by law, I hereby release, waive, discharge, and covenant not to sue Golf Life 24/7, its owners, members, managers, employees, contractors, and agents (collectively, the “Released Parties”) from any and all claims, demands, actions, causes of action, damages, losses, costs, or expenses of any kind arising out of or related to my presence at or use of the facility, including any injury, illness, or property damage that I may suffer, whether caused by the negligence of the Released Parties or otherwise.I further agree to indemnify and hold harmless the Released Parties from and against any and all claims, liabilities, damages, costs, and expenses (including reasonable attorneys’ fees) arising out of or related to my use of the facility, my violation of any rules or instructions, or the actions of any guests I bring to the facility.

8. ACKNOWLEDGMENT AND AGREEMENT

By checking the box on this form and submitting my information, I acknowledge that I have read this Waiver & Release of Liability in full, understand its terms, and agree to be bound by it. I understand that I am giving up substantial legal rights, including the right to sue, and I sign and submit this waiver freely and voluntarily.If I am signing on behalf of a minor, I represent that I am the minor’s parent or legal guardian and that I have the legal authority to agree to this waiver on the minor’s behalf.
